So they want a blank invoice, with no totals, no price per hour, no description? Presumably then, they're going to fill it in themselves, with a stupid fee on it. By the by ..

You have got to get this sorted out for your own sake! If you are working the '30 day notice period' - for which they will have to pay you - it should end tomorrow (I reckon). Have you got your redundancy pay and your P45? If they are not paying you for this so-called notice period you should invoice them.
