Can anyone point me in the right direction to a standard set of terms and conditions a graphic designer should use.

Try the AIGA website. It's a US site but a lot of the information is equally relevant to UK designers and the Resources and Design & Business sections of the website have a lot of useful forms and templates which you can customise for your own needs.
