I'm not going to fill it in I'm afraid, it requires my google email address (it does say not shared though) and well I don't see why an 'anonymous' survey to require my email address.
Plus I can't answer some of the questions, I didn't do a BA, I did a BSc....

I have however fixed the link for those that are ok with that.
