Contact forms

I've been using Jotform for ages, not really ever bothered looking at how it's working, happy to find somewhere i can make contact forms from in a couple of minutes.

I checked my portfolio's XHTML/CSS validation last night, and everything was perfect except the Jotform I put in.

It adds 4/5 HTTP requests as well thanks to links to 3 CSS files hosted on their website and 2 JS files, which is a bit sucky.


Do you know of an equivalent? Or have a contact form layout you'd like to share?
