What needs to go on an Invoice?

Hi Tom,
Welcome back, hope you had a good time in sunny Spain!
My invoice template includes the following information;
My logo top left and my name and address top right.
Invoice to: Client Contact, Company Name, Address
Invoice Date
Invoice Number (My reference number)
Table with 3 columns:
Description of job/project
Hours spent (not always used just if its a per hour job)
Amount
...then at the bottom of the table a total £ payable.
Under that...
Terms: Payment within 30 days.
and details of how payment can be sent
