PHP form resource

Evening all,

I'm being a sad b*stard and spending my Saturday evening working on the redesign on my site. Unfortunately I don't have my utility belt of bloody useful bookmarks of plugins, scripts, etc at home that I have gathered at work.

Can anyone suggest a good opensource code for a php contact form?

Also a decent js tooltip?

i have a good php contact form, and takes spammers IP addresses instead of just rejected stuff.
