If my keyword for example is 'design'...and I have that word on my page 5 or so times, that's fine.

Does it also count if the word is within a word such as designer, designing, designed?

Me neither. I'd just say write naturally and there is no harm in using derivatives or 'design' if that is what the text needs.

If you search on Google you'll find a difference in placings between design and designs, which suggests that they will be treated as different words, though Google only knows...
I've heard that Google doesn't like too many keywords in one place.
