Having had an issue lately where a client point blank refused they told me to proceed with a print job after the sent me a confirmation e-mail! (I had to re-print the job as it had errors on it!!
I've been looking around for something where I could upload a proof and have a client either comment on it with changes or give me approval. Found this site today and signed up to it to try it out: ProofHQ I've had a client test it out with a proof for an A3 NCR set I'm designing for her and its been great. She can leave comments on the artwork specifying changes and also highlight and draw basic shapes. It also keeps a history of everything and all the revisions. Some of you may want to check it out if you don't already have a proofing system in place. ProofHQ

I've been looking for something like this for my workplace, we originally looked into resourcebase but this is far cheaper option (as we have a solution for image sharing too now)! Thanks for sharing....

If you need to server many different clients and easily be able to customise their internal workflows as part of the online proof approval, you should check the Smartflow system Flexible Artwork Approval Solutions from Gap Systems - hope that helps.

Wow, this is an old thread... But in case anyone still looks...
Although this is directed at web designers, I'm sure GetSignOff - Present. Refine. Getsignoff for web designers to manage client feedback could be useful. Another possible solution would be to have a WordPress installation where you upload images and allow clients to comment. You can make this private in the settings.
